Moldovan Birth Certificates for United States born children
If you have a child born in the United States from at least one parent who is Moldovan citizen, you can get a Moldovan birth certificate for your child thus confirming his Moldovan citizenship. Hereafter you can include your child into your Moldovan passport or get a separate passport for him.
Moldovan birth certificates are issued in the United States for children under 1 (one) year.
Documents to be submitted:
- United States birth certificate apostilled by the Secretary of State of the state of birth;
- A medical certificate from the hospital were the child was born confirming the date of birth, place of birth;
- Original passports of both parents;
- Original marriage certificate;
- Copies of the Moldovan internal ID's (Buletin de identitate) with the attachments (if applicable);
- Two passport-size photographs of the child (only if you want your child to be included in your passport. This service cost addidtional $50 for each child included in the passport);
- Questionnaire
- Money order, company check or personal check for $100 made out to Consular Assistance, Inc.
- Self-addressed pre-paid envelope or $18 for overnight DHL return shipping.
